If you turned in your release form when you became a finalist, you won't need to do any more paperwork. They'll use the address you put on there to send your prize.
The amount of time it takes varies a LOT -- some things take maybe a week, some I've been waiting about 10 weeks. If it gets to be more than 8 weeks, you should write to joecool* and make sure it is still on the list to be sent out. But be patient, they're overworked, understaffed, and getting to stuff as fast as they can!
Don't forget to do your intro video REALLY promptly. You have less than 48 hours to turn it in. Have fun, and congrats again!
